2009 Jun - Clinical characteristics and outcomes of Pneumocystis Pneumonia: a 10 year retrospective study

Objective: To evaluate the clinical characteristics and outcome of Pneumocystis pneumonia (PCP) in Hong Kong and to compare the characteristics between HIV-infected and non-HIV infected patients.

Design: Retrospective review of medical records from January 1999 to July 2008 in three regional acute hospitals and one Chest hospital

Patients: A total of 26 HIV-infected patients and 9 otherwise immunosuppressed patients were included. The comparison included epidemiological, clinical, laboratory, radiological, microbiological and clinical outcomes.

Results: The non-HIV PCP patients were mainly constituted by renal transplant recipients that were of higher mean age when compared with HIV-infected patients (51.78 years ± 11.22 vs 42.85 years ± 11.96, p=NS). The mortality was significantly different in the two groups (HIV-infected PCP vs non-HIV PCP: 2/26 patients vs 4/9 patients, p=0.03). ICU admission rate was higher in non-HIV PCP patients than HIV-infected patients (6/9 67% vs 7/26, 27% patients, p=0.05).

Conclusion: There are significant differences in PCP outcomes between HIV-infected PCP patients to non-HIV PCP patients in Hong Kong. Non-HIV infected patients were older, sicker with higher mortality than HIV-infected patients.
