2007 A local study of CPAP adherence and its associated factors in patients with obstructive sleep apnea

Objective
(1) To study the adherence rate of continuous positive airway pressure (CPAP); and
(2) To identify factors associated with its CPAP adherence among patients with obstructive sleep apnea (OSA) in a local hospital

Design
A retrospective study

Subjects
Thirty patients suffering from moderate or severe OSA, who agreed for home CPAP treatment. The CPAP was equipped with monitoring chip for recording the CPAP usage.

Setting
Sleep unit of a Chest hospital in Kowloon.

Measurements
Subjects’ demographic data, clinical progress and result of the polysomnogram were obtained from the hospital in-patient and out-patient records. Data on CPAP usage was collected through the internet for the 3 months after commencement of home CPAP.

Results
Thirty subjects were recruited in the study. Eighteen of them (60%) were considered as having good adherence according to the recommendation of American Academy of Sleep Medicine (AASM). They were found to have earlier follow-up visits (05.50  02.64 weeks vs 10.58  06.75 weeks, p=0.007) when compared with those labeled as poor adherence. A higher proportion of subjects with good adherence did not develop side effect (55.6% vs 0%, p=0.002) associated with the use of CPAP when compared with their counterpart.

Conclusion
Subjects who attended first follow-up earlier and subjects who were free from side effects related to CPAP use were associated with good CPAP adherence.
